Schedule: 8-5 / Monday - Friday

We are seeking a compassionate resident-focused Social Worker to join our Skilled Nursing Facility this role you will advocate for residents support families and help ensure the highest quality of care and quality of life for those we serve.

Position Summary

The Social Worker is responsible for assessing residents psychosocial needs coordinating services facilitating care planning and providing emotional support to residents and families throughout their stay. This position works collaboratively with the interdisciplinary team to promote resident well-being successful transitions of care and compliance with regulatory requirements.

Qualifications

Qualified applicants are required to possess 2 years related experience with at least 1 year of experience in a health care setting (hospital clinic rehabilitation center etc.).

Bachelors Degree in Social Work Psychology Gerontology or Sociology from an accredited institution is required.

Additional requirements include excellent problem solving skills using a customer-focused approach to problem solving and goal setting excellent communication and listening skills enthusiastic resourceful organized and possesses strong personal awareness of nonverbal communication. Computer skills including proficiency in MS Office.
