Job Summary
A 64-bed adult psychiatric facility Lasting Hope Recovery Center is a place of healing. All of the rooms are private. At Lasting Hope Recovery Center patients find comfort and a team of caring professionals working for them. Often mental illness and substance abuse occur at the same time. CHI Health Lasting Hope Recovery Center offers an entire range of treatment and prevention services to meet the needs of patients experiencing both challenges. Treatment is built on personal strengths and support systems that treat individuals experiencing these illnesses.
As our Social Worker youll play a vital role in helping patients and families navigate complex healthcare needs with compassion and expertise. By addressing social emotional financial and discharge planning needs youll help remove barriers to care and support positive patient outcomes throughout the care journey.
Every day you will collaborate with patients families physicians nurses and the interdisciplinary care team to develop individualized plans of care and coordinate safe timely transitions. Youll serve as a trusted advocate connecting patients with community resources and support services that promote continued health and well-being beyond the hospital.

- Assess patients psychosocial environmental financial and discharge planning needs to develop individualized plans of care.
- Collaborate with the interdisciplinary care team to coordinate safe effective discharge plans and facilitate timely referrals to appropriate post-acute and community resources.
- Advocate for patients and families by connecting them with support services and assisting with applications for available community and financial resources.
- Educate patients families and healthcare team members on available services care options and community resources to support successful transitions of care.
- Document assessments interventions and care coordination activities while maintaining compliance with organizational state and federal standards.

- Certified Social Worker (CSW) in Nebraska
- Minimum of a bachelors degree in Social Work
- Basic Life Support (BLS) from the American Heart Association
