Responsibilities:
- Manage and support the social services department to provide exceptional care and service to residents.
- Conduct assessments and develop individualized care plans in coordination with clinical teams.
- Guide residents and families through admission adjustment and discharge processes.
- Ensure compliance with federal state and facility policies related to resident care and rights.
- Maintain accurate documentation and participate in care plan meetings.
Qualifications:
- Bachelors degree in Social Work (BSW) or related field required.
- Current Connecticut licensure (LSW or LMSW) preferred.
- Previous experience in a skilled nursing or long-term care setting strongly desired.
- Excellent communication organization and leadership skills.
