Principal Responsibilities

• Plan organize implement and evaluate a comprehensive Social Services program
• Prepare the Social Services department for annual surveys and inspections
• Collaborate with the interdisciplinary team to promote resident rights and psychological well-being
• Complete social historiespsychosocial assessments and relevant MDS sections to guide care planning
• Maintain accurate timely documentation in compliance with federal state and company requirements
• Participate in the development and implementation of interdisciplinary care plans
• Provide therapeutic interventions to assist residents with adjustment to long-term care
• Support residents right to self-determination and document refusals and alternative options
• Educate residents and families on advance directives resident rights and available community resources
• Coordinate discharge planning including referrals for home care and post-discharge services
• Initiate and participate in Discharge Transition Plans and discharge education
• Maintain community agency relationships and referral resources
• Participate in quality assurance and interdisciplinary team meetings
• Educate staff onpsychosocial needs cultural diversity resident rights and abuse prevention
• Identify and report suspected abuse neglect or exploitation to Adult Protective Services (APS)
• Collaborate with nursing leadership and administration to prepare for surveys and inspections
• Provide clinical support during catastrophic or crisis events
• Perform other duties as assigned to support resident well-being and facility operations

Qualifications

• Associate degree in a human services field or related social services experience (Bachelors degree preferred)
• Minimum ofone year of experience in a human services role within a hospital skilled nursing or medical facility
• Willingness to obtainSocial Services Designee certification if not already obtained
• Knowledge of resident rightspsychosocial needs and long-term care regulations
• Strong communication documentation and collaboration skills
