Job Summary


The SMI Rehabilitation Specialist provides psychiatric rehabilitation vocational support and skills-based services to individuals enrolled in Valle del Sols Serious Mental Illness (SMI) program. Working collaboratively with the interdisciplinary treatment team this position assists members in identifying strengths developing independent living skills achieving employment and educational goals accessing community resources and increasing meaningful participation in community life.

Using a strengths-based trauma-informed culturally responsive recovery-oriented and person-centered approach the SMI Rehabilitation Specialist develops and implements individualized rehabilitation interventions that promote recovery independence community integration and improved quality of life.


Psychiatric Rehabilitation Services

  • Conduct psychiatric rehabilitation assessments to identify member strengths interests preferences barriers and rehabilitation goals.
  • Utilize approved assessment tools including the Vocational Activity Profile (VAP) or other organizationally approved instruments to support individualized rehabilitation planning.
  • Develop and implement individualized rehabilitation interventions that promote recovery independent living education employment social engagement and meaningful community participation.
  • Provide individual and group rehabilitation services that support skill development role achievement and increased independence.
  • Assist members in developing skills necessary to achieve rehabilitation goals identified within the Individual Service Plan (ISP).

Vocational Rehabilitation & Community Integration

  • Identify members who may benefit from vocational rehabilitation supported employment educational opportunities and other meaningful community activities.
  • Coordinate timely referrals to vocational rehabilitation agencies supported employment providers educational resources and community-based organizations.
  • Monitor member participation and progress in rehabilitation employment education and community integration activities.
  • Develop collaborative relationships with employers vocational providers educational institutions and community organizations that support member success.
  • Educate members regarding employment opportunities work incentives benefits planning psychiatric rehabilitation services and community resources.

Care Coordination & Interdisciplinary Collaboration

  • Collaborate with behavioral health and medical providers case managers care coordinators peer support specialists families and community partners to coordinate rehabilitation services.
  • Participate in interdisciplinary treatment team meetings and contribute to the development implementation and ongoing review of Individual Service Plans (ISPs).
  • Provide consultation education and technical support to team members regarding psychiatric rehabilitation principles vocational services and available community resources.
  • Coordinate services that support members in achieving rehabilitation educational employment housing and community integration goals.

Documentation Quality & Program Performance

  • Maintain accurate timely and complete documentation in accordance with organizational policies payer requirements and regulatory standards.
  • Maintain current demographic educational employment and rehabilitation information to support service planning care coordination and reporting.
  • Monitor assigned performance measures productivity expectations and quality indicators.
  • Participate in quality improvement initiatives program evaluations required meetings and performance improvement activities.

QUALIFICATIONS

Education

Required:

  • Bachelors or Masters degree in a behavioral health-related field from an accredited institution; OR
  • Bachelors or Masters degree in a non-behavioral health field from an accredited institution and one (1) year of behavioral health experience; OR
  • Associate degree from an accredited institution and two (2) years of behavioral health experience; OR
  • High School diploma/GED and four (4) years of behavioral health experience including one (1) year of direct patient contact.

Preferred:

  • Counseling Marriage and Family Therapy Social Work Addiction Counseling or a related behavioral health field from an accredited institution.

Experience

Required:

  • Experience providing behavioral health psychiatric rehabilitation recovery support vocational services or related human services.
  • Experience working with adults diagnosed with Serious Mental Illness (SMI) or other complex behavioral health conditions.
  • Experience collaborating within interdisciplinary healthcare or behavioral health teams.

Preferred:

  • Experience with supported employment vocational rehabilitation or psychiatric rehabilitation programs.
  • Experience utilizing evidence-based psychiatric rehabilitation models.
  • Experience within an integrated healthcare or Federally Qualified Health Center (FQHC) environment.

Licensure/Certification

Required:

  • Must meet qualifications for and obtain Behavioral Health Technician (BHT) credentialing within 90 days of hire and maintain credentialing requirements throughout employment.
  • Must meet the licensure certification or credentialing requirements applicable to the position in accordance with Arizona law and applicable regulatory authorities.
  • Must practice within the scope of applicable credentials consistent with organizational policies and the needs of the populations served.
  • BLS certification or ability to obtain within 5 days of start date.
  • Arizona Level I Fingerprint Clearance Card or ability to obtain within 5 days of start date.

Preferred:

  • Peer Support Certification.
  • Chronic Disease Self-Management training.
  • Specialized training related to SMI High Needs Early Childhood or integrated care populations.
  • Specialized training related to integrated care social determinants of health patient navigation care transitions or community resource coordination.

Other Requirements

  • Must be at least 18 years of age.
  • Must be authorized to work in the United States.
  • Must be able to pass pre- and continued employment screenings successfully.
  • Access to reliable transportation.
  • Must possess and maintain a valid drivers license an acceptable driving record and meet Valle del Sols insurance requirements when driving a company vehicle.


KNOWLEDGE SKILLS AND ABILITIES

  • Knowledge of psychiatric rehabilitation recovery principles supported employment vocational rehabilitation and community-based behavioral health services.
  • Knowledge of Serious Mental Illness (SMI) behavioral health systems community resources and rehabilitation service delivery.
  • Knowledge of evidence-based psychiatric rehabilitation practices recovery-oriented care and person-centered planning.
  • Ability to conduct rehabilitation assessments and develop individualized rehabilitation interventions.
  • Ability to coordinate services and collaborate effectively with interdisciplinary teams community providers and vocational partners.
  • Ability to establish and maintain effective therapeutic relationships with members families and community stakeholders.
  • Strong communication organization problem-solving and documentation skills.
  • Ability to manage multiple priorities while maintaining quality and compliance standards.
  • Proficiency in Microsoft Office Suite and electronic health record (EHR) systems.
  • Ability to communicate effectively with members families staff and community partners.
  • Ability to maintain accurate documentation and comply with regulatory requirements.
  • Ability to work effectively within multidisciplinary and integrated care environments.
  • Ability to provide culturally responsive trauma-informed recovery-oriented and person-centered services.


WORKING CONDITIONS & PHYSICAL REQUIREMENTS

Work Environment

This position may operate in clinical office or community settings including clinics administrative offices outreach locations client homes partner agencies and community sites. Employees may interact with individuals experiencing behavioral health challenges emotional distress or complex social needs. Standard safety precautions and personal protective equipment (PPE) may be required as applicable.

Schedule & Travel

Work schedules may vary based on program needs and may include evening or weekend hours. Some positions may require local travel between Valle del Sol sites or community locations.

COMPLIANCE REQUIREMENTS

Employees must maintain compliance with all Valle del Sol policies and applicable regulatory requirements which may include:

  • Licensure certification credentialing and/or privileging requirements applicable to the position.
  • Required immunizations and occupational health clearances.
  • Tuberculosis (TB) testing and Hepatitis B screening or declination when applicable.
  • Arizona Level I Fingerprint Clearance Card when required by law or program.
  • Valid drivers license and acceptable driving record when operating a vehicle for company business.
  • Completion and maintenance of required training certifications and continuing education.

Job Qualifications:

  • SOAR certification preferred or willingness to obtain upon hire.
