Job Summary
Adelphi is seeking a Licensed Independent Social Worker (LISW) and/or Licensed Clinical Social Worker (LCSW) to provide school-based social work evaluations direct and indirect services consultation and related student support services within a school setting in Albuquerque New Mexico. The Social Worker will work collaboratively with students families educators and multidisciplinary teams to address student needs identified through the special education and IEP process.

Requirements
Current State of New Mexico Board of Social Work Examiners license to practice as a Licensed Independent Social Worker (LISW) and/or Licensed Clinical Social Worker (LCSW).
Current State of New Mexico Board of Social Work Examiners Supervisor Certification with the providers name appearing on the applicable NMBSWE supervisor listing.
Current New Mexico Public Education Department licensure for Social Workers.
Completed application for a Medicaid Provider Number or an assigned Medicaid Provider Number.
Appropriate education training and professional preparation necessary to provide school-based social work services.
Responsibilities
Conduct social work evaluations for eligible assigned students when requested.
Provide direct and indirect school-based social work services in accordance with applicable state and federal regulations.
Design and implement social work services appropriate to individual student needs and IEP requirements.
Provide consultation to teachers and other members of the students educational team.
Participate in multidisciplinary and IEP-related planning and service coordination as appropriate.
Provide supervision to Licensed Master Social Workers in accordance with NMBSWE requirements when requested.
Maintain assigned caseload information and accurate student service records.
Prepare and submit progress reports evaluation reports and other required documentation within established timelines.
Complete Medicaid service documentation in accordance with applicable District procedures.
Complete federal and state documentation governing the provision of social work services.
Provide monthly billing and student contact forms as required.
Collaborate with parents teachers administrators special education personnel and other professionals involved in student services.
Coordinate service schedules with designated Special Education and school-site representatives.
Provide services in accordance with professional standards of practice and ethical requirements.
Maintain strict confidentiality of student family and District information.
Provide services only for assignments properly authorized through the designated Special Education Department.
Maintain required licensure certifications and compliance documentation throughout the assignment.
Perform other duties as assigned.
