Patient Care Manager Fall River
Schedule: Full-time Monday - Friday 8 a.m. to 5 p.m.
Key Responsibilities
- Provide clinical supervision to ensure patient care meets professional standards agency policies and regulatory requirements
- Coordinate and monitor patient care delivery
- Supervise clinical staff to ensure consistent appropriate service
- Maintain clear communication with patients caregivers referral sources and field and office staff
- Drive quality outcomes by meeting department goals and supporting improvement initiatives
