What Youll Do

• Complete start-of-care recertification and discharge visits including OASIS assessments
• Build individualized plans of care that help patients reach their goals
• Coordinate with physicians therapists and your LPN partner
• Teach patients and families the skills to thrive at home
• Document accurately and on time

What Youll Need

• Active Oklahoma RN license
• Valid drivers license and a clean driving record
• Home health experience preferred but if you have strong clinical skills and a heart for patients well teach you the rest
• OASIS experience is a plus
