RN- Long Term Care (LTC)

We are seeking an experienced Registered Nurse (RN) to provide safe compassionate and high-quality patient care in a Long-Term Care/Skilled Nursing setting in Harvard Illinois.

The RN will use clinical judgment and the nursing process to assess patients develop and implement individualized care plans administer treatments and collaborate with physicians and interdisciplinary healthcare teams.

• Perform comprehensive patient assessments and monitor changes in condition.
• Develop implement and evaluate individualized care plans.
• Administer medications and treatments according to orders policies and applicable regulations.
• Document patient care accurately and in a timely manner using Epic EMR.
• Identify and promptly escalate changes in patient condition or safety concerns.
• Provide patient and family education regarding medications treatments and care plans.
• Collaborate with physicians nurses therapists and other healthcare professionals.
• Delegate appropriate tasks to nursing and support staff while maintaining accountability.
• Promote patient safety quality care and positive patient outcomes.
• Participate in quality improvement professional development and continuing education activities.
• Follow all facility policies nursing standards and regulatory requirements.
• Perform other nursing duties as assigned.

• Graduate of an accredited nursing program.
• Current and active Illinois RN License.
• Current BLS/BCLS certification.
• LTC SNF or similar clinical experience preferred.
• Strong clinical assessment critical-thinking and organizational skills.
• Excellent communication and teamwork abilities.
• Ability to safely administer medications and treatments.
• Education credentials must be verifiable.
