Requirements:
- Active Registered Nurse (RN) license
- Graduate of an accredited nursing program (ADN or BSN)
- Minimum of 1 year of RN experience; hospice palliative oncology or med-surg experience preferred
- Strong assessment critical thinking and communication skills
- Ability to provide compassionate care in emotionally challenging situations
- Ability to work independently and as part of a multidisciplinary team
Responsibilities:
- Assess patient physical emotional and psychosocial needs
- Develop implement and evaluate individualized plans of care
- Manage pain and symptoms in accordance with physician orders and care plans
- Administer medications treatments and interventions safely and accurately
- Provide education and support to patients and families regarding disease progression care plans and end-of-life needs
- Coordinate care with physicians social workers chaplains aides and other team members
- Monitor patient status and report changes promptly
- Document assessments care and patient responses accurately and timely
- Participate in interdisciplinary team meetings and care planning
- Provide emotional support and guidance to patients and families during end-of-life care
