• Conduct comprehensive patient assessments and develop individualized care plans for home health clients
• Coordinate and manage all aspects of patient care including scheduling referrals and interdisciplinary team communication
• Perform skilled nursing visits monitor patient progress and adjust care plans as clinically indicated
• Educate patients and family members on diagnoses medications and self-care techniques
• Collaborate with physicians therapists and other healthcare professionals to ensure continuity of care
• Maintain accurate and timely clinical documentation in compliance with regulatory and agency standards
• Ensure patient safety and advocate for optimal health outcomes throughout the care continuum

• Active RN license in the state of Colorado (or compact license eligibility)
• Previous home health case management or community health nursing experience preferred
• Strong clinical assessment and critical thinking skills
• Excellent communication organization and time management abilities
• Proficiency with electronic health records (EHR) and clinical documentation
• Valid drivers license reliable transportation and willingness to travel within Northern CO service area
• Compassionate patient-centered approach with a commitment to quality care
