RN HH Case Manager

Location: Burlington & Walworth Wisconsin
Department: Hospice Home Health
Shift: Day Shift (Monday Friday 8:00 AM 4:30 PM)

We are seeking a compassionate and experienced Registered Nurse (RN) Hospice Case Manager to join a dedicated hospice team serving patients in the Burlington and Walworth communities.

The RN Case Manager will coordinate and manage care for patients with end-stage illnesses ensuring comfort-focused care in various home-based settings. This role requires strong clinical judgment excellent communication skills and the ability to provide holistic care to patients and their families.

• End-stage hospice patients

• Care provided in:

  • Patient homes

  • Assisted Living Facilities (ALF)

  • Skilled Nursing Facilities (SNF)

  • Group homes

  • Independent living communities

  • Two hospitals providing General Inpatient (GIP) hospice care

• Average Daily Census: 105 patients

• RN Case Managers: 7 (each managing 1416 patients)

• CNA Support: 6 CNAs

• Manage a caseload of hospice patients in the community

• Provide comprehensive nursing assessments and symptom management

• Develop and implement individualized care plans

• Coordinate care with physicians hospice staff and interdisciplinary teams

• Educate patients and families regarding disease progression and end-of-life care

• Document patient care accurately in the EMR system

• Provide compassionate support to patients and families during end-of-life care

• Minimum Experience: 2 years RN experience

• Travelers: Experienced travelers preferred

• Licensure: Active Registered Nurse (RN) license

• Certifications:

  • BLS (Basic Life Support)

• Other Requirements:

  • Valid Drivers License

  • Auto Insurance

  • Reliable transportation for community visits

• Hospice or end-of-life care experience preferred

• Strong patient assessment and care coordination skills

• Ability to work independently in a community-based setting

• Excellent communication and compassionate patient care

• EMR: Epic & Rover

• IV Pump: Yes

• Medication Dispensing: Yes

• Monitoring Equipment: Chair and bed alarms

• Supplies: Home health bag and trunk supplies

• Shift: Monday Friday 8:00 AM 4:30 PM

• Weekend Requirement: None

• On-Call: None

• Holiday Requirement: None

• Floating: Community-based patient home visits

• Orientation: 1 week
