RN Case Manager

A leading home health organization in San Marcos California is seeking an RN Case Manager to join its growing clinical team!

Pay: $55.00 - $65.00/hour

RN Case Manager Opportunity:

• Full-time permanent direct-hire position
  
• Monday-Friday 8:00 AM - 5:00 PM schedule
  
• Weekend and after-hours on-call rotations
  
• $12000 sign-on bonus available

RN Case Manager Responsibilities:

• Complete patient assessments and determine home care needs
  
• Develop and revise individualized plans of care
  
• Administer medications and treatments as ordered by physicians
  
• Educate patients and families regarding treatment plans and healthcare needs
  
• Coordinate care with physicians and interdisciplinary healthcare teams
  

RN Case Manager Qualifications:

• Current California RN license required
  
• Minimum of one year of recent acute care experience required
  
• Current BLS/CPR certification required
  
• Valid drivers license and auto insurance required
  
• Home health experience preferred
