RN Case Manager

A leading home health organization in Portland Oregon is seeking an RN Case Manager to join its growing clinical team!

Pay: $55.00 - $60.00/hour

RN Case Manager Opportunity:

• Full-time permanent direct-hire position
  
• Monday-Friday 8:00 AM - 5:00 PM schedule with weekend rotations
  
• Guaranteed weekly pay

RN Case Manager Responsibilities:

• Complete initial patient assessments and develop comprehensive plans of care
  
• Regularly evaluate patient conditions and revise care plans as needed
  
• Utilize assessment findings to establish nursing diagnoses and treatment goals
  
• Administer medications and treatments as prescribed by physicians
  
• Educate patients and families regarding care plans treatments and health management
  
• Coordinate discharge planning and ensure continuity of care
  

RN Case Manager Qualifications:

• Current Oregon Registered Nurse (RN) license required
  
• Minimum of one year of recent acute care experience required
  
• Current BLS/CPR certification required
  
• Valid drivers license and auto insurance required
  
• Associates or Bachelors Degree in Nursing required
  
• Home health experience preferred
