RN Case Manager Houston
Private Duty In-Home Care: Cambridge Caregivers is a family-owned and operated provider of non-medical caregiver services. We are known for personalized attention and our great staff. Our caregivers are W-2 employees who have completed extensive training developed by our Director of Nursing. We have 300 Caregivers CNAs LVNs RNs and EMTs on staff in Dallas Ft. Worth and Houston.

Registered Nurse Responsibilities:

  • Perform comprehensive patient assessments and document findings

  • Monitor and record vital signs (blood pressure temperature heart rate etc.)

  • Administer medications as ordered

  • Provide wound care including dressing changes and monitoring healing

  • Educate patients and families on care plans medications and home care strategies

  • Respond to patient concerns with professionalism and empathy

  • Collaborate with Director of Nursingregardingchanges in patient condition

  • Perform ongoing patient data collection andmaintainaccuratedocumentation

  • Develop update and follow individualized care plans

  • Ensure all orders are followed documented and communicated appropriately

  • Assistwith emergency response measures when necessary

  • Maintain a clean and safe environment

  • Build trust and rapport with patients and families

  • Deliver exceptional patient experience through responsiveness and care

  • Serve as a resource and mentor to patient care staff

  • Pursue ongoing professional development

  • Support and uphold the mission values and philosophy of Cambridge Caregivers

  • Adhere to all company policies procedures and standards of care

Registered Nurse Requirements:

  • Current RN license in the state of Texas and/or compact license

  • Current CPR Certification (American Heart Association)

  • Basic Life Support (BLS) Certification

  • Reliable transportation

  • Ability to work independently and follow clinical direction

  • Compassionate professional and patient focused demeanor
