Purpose: Responsible for reviewing and processing requests for release of information for customers patients physician offices and requestors coming to the department.
Essential Responsibilities:
- Reviews written requests for copies of PHI ensuring compliance in accordance with NYS and HIPAA guidelines.
- Accesses multiple computer applications to obtain demographic and visit information.
- Enters data requests through ROI software for tracking purposes and provides request to copy service for completion.
- Responsible for front desk functions including but not limited to greeting customers assisting with completion of authorization forms informing customers of relevant processes and procedures customer identity verification and fulfillment of high volume fax requests for PHI.
- Performs all necessary tasks associated with off-site storage of paper records including retrieval returns proper labeling utilizing software and knowledge of inventory and retention.
- Participates in department performance improvement activities department meetings and hospital activities as appropriate.
- Ensures work area is organized neat/clean and all patient records are appropriately stored at the end of shift.
- Responds to high volume of telephone calls and voicemails in a timely and courteous manner.

- Performs other duties as required.
Education and Experience Requirements:
- High School diploma or equivalent experience.
- PREFER: Minimum of two (2) years medical record experience with basic knowledge of HIPAA.
Minimum Knowledge Skills and Abilities Requirements:
- Proficient in Microsoft Office including Word Excel and Outlook
- Proficient Internet Navigation
- Strong attention to detail.
- Fax / Copy machine
- Organized and has the ability to prioritize requests.
