Responsibilities include but not limited to:

• Identifying medical needs for individual members and setting care priorities accordingly
• Developing ongoing Plans of Care in partnership with Senior LIFE interdisciplinary teams
• Interpreting recording and acting on diagnostic results and performing advanced nursing techniques and critical care procedures
• Identifying and assessing major medical problems and communicating potential needs to a physician or specialist

Qualifications

Applicants should have at least one year working specifically with an elderly or aging population hold a current RN license and valid drivers license.
