The first home health visit sets the foundation for a patients care. Your assessment clinical judgment and guidance can help patients and families understand what comes next.

Elara Caring is seeking a Registered Nurse primarily focused on Start of Care visits in Mentor Painesville Chardon Ashtabula Geneva and surrounding communities.

Your Role

• Complete comprehensive Start of Care assessments to identify patient needs and establish a foundation for individualized care.
• Complete ongoing assessments and provide nursing care as assigned.
• Provide care according to the plan of care and physician orders.
• Monitor patient conditions communicate changes and take appropriate action.
• Help patients and families understand their care safety needs and when to seek assistance.
• Coordinate with physicians and the care team to support a smooth transition into home health.
• Maintain accurate clinical documentation and protect patient and staff privacy.
• Support safe high-quality continuous patient care.

What Youll Need

• Current RN license valid in Ohio.
• Graduation from an accredited certificate diploma associate or bachelors nursing program.
• At least 1 year of nursing experience in a clinical care setting.
• Home health experience preferred but not required.
• Ability to frequently lift push pull and support up to 50 pounds including positioning or transferring patients and moving equipment.
• Must possess a valid drivers license maintain an acceptable driving record OR have reliable transportation to travel between patient homes and assigned work locations.
