Qualifications
Your responsibilities include:
- Develop and implement individualized care plans based on current and prior medical history
- Deliver attentive one-on-one support to patients and their families ensuring comfort dignity and understanding
- Conduct thorough assessments of physical emotional and spiritual needs throughout the care journey
- Accurately monitor and document vital signs including temperature pulse respiration and blood pressure
- Administer physician-ordered medications and treatments with skill and empathy
- Educate patients and their families on care techniques and symptom management
- Track and communicate changes in patient condition ensuring timely and effective responses
- Maintain detailed and accurate clinical documentation reflecting patient progress and quality of care
- Collaborate with interdisciplinary team members to ensure comprehensive care delivery
- Participate in on-call duties as defined by the on-call policy
- Perform additional duties as assigned in support of patient care and team success
What You Bring:
- A heart for hospice care and a commitment to exceptional service
- A valid Registered Nurse (RN) license in good standing in the state of Hawaii
- Current CPR certification
- A valid drivers license and reliable transportation
- Strong observational documentation and communication skills
- A flexible collaborative and patient-focused approach to care
