As a Home Health Registered Nurse youll provide individualized care in your patients homes helping them recover manage chronic conditions prevent hospitalizations and remain safely where they want to beat home.

- Perform comprehensive patient assessments
- Develop and implement individualized plans of care
- Provide skilled nursing services medication management wound care and patient education
- Monitor patient progress and communicate changes to physicians and the interdisciplinary team
- Coordinate care with therapists social workers and home health aides
- Complete timely and accurate clinical documentation
- Support patients and families throughout their healthcare journey
- Active Maryland Registered Nurse (RN) License Required
- Valid Drivers License and Reliable Transportation
- Strong Clinical Assessment and Documentation Skills
- Excellent Communication and Organizational Skills
- Home Health Experience Preferred but Not Required
- Experience with OASIS wound care case management or chronic disease management is a plus
