• Complete patient assessments and referral screenings.
• Gather clinical and demographic information and determine admission appropriateness.
• Coordinate admissions including insurance verification authorizations physician orders and required documentation.
• Communicate with patients families providers and referral sources throughout the admission process.
• Maintain referral tracking and support census and bed management efforts.
• Serve as a patient advocate and provide ongoing support to patients and families.
• Build relationships with community partners and educate referral sources on admission criteria and services.
• Assist with psychosocial interventions and patient groups as needed.

• Current RN license in the state of practice.
• BLS and Handle With Care (HWC) certification within 45 days of hire.
• Strong assessment communication and care coordination skills.
