Job Summary

PURPOSE:The Bruner House Community Residence (Bronx NY) provides family-oriented and supportive Child and Teen Services to boys and girls ages of 13-15 years old who are struggling with a wide range of psychiatric diagnoses emotional crises mental health and behavioral issues. Under their individualized service plans youth receive comprehensive clinical treatment and education and are able to develop independent living skills and adaptive coping strategies for managing of their emotional and behavioral issues in order transition back into their home OVERVIEW:A registered nurse (RN) assigned to a facility is responsible for the complete nursing care and the performing of all routine medical procedures within the scope of the nursing practice for the clients under his/her charge. The RN generally reports to the Nursing Supervisor however in all matters of medication is supervised by the attending physician or administers the medication in line with an external physicians include but are not limited to: Performing nursing assessments of residents upon program entry and annually thereafter; determining the residents general health condition identify any risk factors and providing care for any presenting/out-standing medical issues; providing referrals for follow-up treatments; developing a treatment care plan based on residents needs; responding to medical emergencies immediately upon notification; performing triage and providing access to all necessary medical treatment; following up to ensure complete recoveryAdministering prescribed and/or routine medical treatments in line with doctors orders and internal agency policies; scheduling clinic appointments; escorting residents to appointments; running medication groups as neededProviding health education and communicating information regarding good health practices to the residentsLiaising between external medical health care providers schools other community organizations and the facility with regards to the clients medical needs; performing all necessary charting and other patient documentation; monitoring lab results for change and communicating change(s) to the physicianPerforming comprehensive examination of any resident involved in a restraint; providing advocacy for hospitalized clientsOther duties as assignedRequirementsBachelors in nursing with New York State registration is required; OR RNExperience in a residential setting or in the mental health field is strongly preferredCORE COMPETENCIESTranslate research and other forms of knowledge to improve practice processes and outcomesCommunicate practice knowledge effectively both orally and in writingparticipate in agency quality improvement projects to ensure best client outcomesCreate a climate of patient-centered care to include confidentiality privacy comfort emotional support mutual trust and respect. EXPERIENCE REQUIRED/LANGUAGE PREFERENCE:Excellent oral writing communication and organizational SKILLS REQUIRED: Proficiency in Microsoft Office Suite applications AND MANUAL DEXTERITY:The candidate should be able to read documents for analytical purposes such as health charts spreadsheets the manual dexterity necessary for handling medical equipment administering medications and performing clinical ENVIRONMENT/PHYSICAL EFFORTAbility to walk sit stand and bend as necessary to assess and provide care to clients effectively in various be capable of responding swiftly to emergencies which may involve moving quickly to the scene performing CPR and utilizing appropriate emergency in lifting transferring and repositioning clients safely when needed ensuring the comfort and safety of both clients and the physical stamina to perform nursing duties throughout the shift including prolonged periods of standing and walking while remaining attentive to patient to navigate a variety of environments including tight spaces while maintaining a focus on patient care and safety.
