Job Description Summary

We are seeking a Registered Nurse (RN) to join our Home Health Team in Collier this role there is a real impact in the comfort of patients homes. As part of our compassionate highperforming care team youll provide skilled patient-centered home health care through assessments care planning clinical interventions patient education and collaboration with the care team while maintaining accurate documentation and compliance with regulatory standards.

This is a Pro Re Nata (PRN) position with variable time and hours can vary from week to week based on staffing needs patient volume call-offs vacations or organizational requirements.

How will you make an impact & Requirements

• Patient Assessment: Conduct comprehensive assessments of patients physical mental and emotional health including vital signs medication administration wound care and disease management.
• Care Planning: Develop individualized care plans that address patients specific needs and goals including medication administration wound care and disease management.
• Patient Education: Educate patients and their families on disease management medication administration and self-care techniques to promote independence and improve health outcomes.
• Clinical Interventions: Provide direct patient care including wound care medication administration injections and IV therapy.
• Collaboration: Collaborate with physicians therapists and other healthcare professionals to ensure coordinated and comprehensive care.
• Documentation: Maintain accurate and timely documentation of patient assessments interventions and progress notes.
• Compliance: Adhere to all relevant regulatory standards including HIPAA and Medicare regulations.

• Licensure: Current and valid Registered Nurse (RN) license in the state of Florida.
• Experience: Minimum of 1 year of experience in a hospital or skilled nursing facility.
• Skills: Strong clinical skills excellent communication and interpersonal abilities and a patient-centered approach to care.
• Professionalism: Strong work ethic reliability and a commitment to providing high-quality care.
• Transportation: Reliable transportation to visit patients in their homes.
