Responsibilities:
- Provide skilled nursing care case management and specialized care to patients.
- Perform comprehensive daily patient assessments.
- Administer medications safely and effectively.
- Develop implement and manage individualized treatment plans.
- Collaborate with an interdisciplinary healthcare team to coordinate patient care.
- Maintain strong communication with patients healthcare providers and team members.
- Travel to various locations to provide field-based care.
- Provide skilled nursing care case management and specialized care to patients.

Required Skills & Certifications:
- Active Registered Nurse (RN) license in the state of practice.
- Minimum of 2 years of Labor & Delivery (L&D) experience.
- Minimum of 2 years of hospital home health or hospice care experience.
- Basic Life Support (BLS) certification.
- Advanced Cardiovascular Life Support (ACLS) certification.
- Neonatal Resuscitation Program (NRP) certification.
- Bachelors degree in Nursing or a related field.
- Valid drivers license active car insurance and a reliable automobile for field work.
- Strong clinical assessment medication administration and treatment planning skills.
- Active Registered Nurse (RN) license in the state of practice.
