We are seeking a Care Coordinator RN for a 13-week contract opportunity in Chico California. This role is responsible for coordinating patient care across the continuum collaborating with multidisciplinary teams optimizing resource utilization and ensuring high-quality cost-effective care throughout each patients episode of treatment.
Responsibilities
Coordinate patient care plans in collaboration with physicians patients families payors and the interdisciplinary care team
Perform comprehensive patient assessments care planning intervention and evaluation using the nursing process
Advocate for patients and families while ensuring effective communication throughout the episode of care
Document patient care activities accurately and support care coordination using EPIC EMR
Requirements
Active California RN License required
Minimum 1 year of recent Case Management experience in an acute care setting required
Current BLS/BCLS Certification required
Experience with EPIC EMR preferred
