Job Summary

Position: Registered Nurse Case Manager Company: Ljwleding Location: California City CA USA Contract Details: Full-time Ljwleding is seeking a highly motivated and compassionate Registered Nurse Case Manager to join our team in California City CA. As a leading healthcare provider our mission is to provide exceptional care to our patients and their families and we are looking for a dedicated RN Case Manager to help us achieve this goal. As an RN Case Manager you will be responsible for coordinating and managing the care of patients with complex medical needs. You will work closely with physicians nurses and other healthcare professionals to develop and implement individualized care plans for each patient. Your role will also involve advocating for patients and their families ensuring that they receive the best possible care and support. Responsibilities: - Conduct comprehensive assessments of patients medical social and emotional needs - Develop and implement individualized care plans in collaboration with the patient family and healthcare team - Coordinate and manage all aspects of patient care including scheduling appointments arranging for home health services and coordinating referrals to specialists - Monitor patients progress and make adjustments to the care plan as needed - Educate patients and their families about their condition treatment options and self-care techniques - Collaborate with community resources to provide support and resources for patients and their families - Keep accurate and up-to-date patient records and documentation - Communicate regularly with physicians and other healthcare professionals to ensure coordinated and efficient care - Participate in interdisciplinary team meetings to discuss patient care and identify areas for improvement - Adhere to all legal and ethical standards of nursing practice Requirements: - Active RN license in the state of California - Minimum of 2 years of experience in case management or a related field - Strong clinical assessment and critical thinking skills - Excellent communication and interpersonal skills - Ability to work independently and as part of a team - Proficient in computer skills and electronic medical records - Knowledge of Medicare and Medicaid regulations and guidelines - Bilingual in Spanish is a plus We offer a competitive salary and benefits package as well as opportunities for professional development and growth.
