How youll contribute

A Case Manager RN who excels in this role:

• Monitors and manages patient care to promote continuity optimal outcomes satisfaction cost efficiency and regulatory compliance.
• Consults regularly with nursing staff and the multidisciplinary team to evaluate patient status and appropriateness of care including admission length of stay transfers and discharge.
• Leads and coordinates discharge planning including transfers to other facilities referrals to community resources and discharge education.
• Monitors patient and family satisfaction; responds to questions and concerns from patients families and payors.
• Performs chart reviews to identify and address issues related to service delivery outcomes compliance cost and reimbursement.
• Communicates effectively with providers payors and ancillary services to remove barriers and ensure timely care progression.
• Accurately documents case management activities plans and outcomes.
• Participates in performance improvement and required continuing education.
• Supports department operations by working alternate shifts/units as needed; may be assigned oncall.
• Performs other duties as assigned.

What were looking for

Education & Licensure

• Associates Degree in Nursing required
• Bachelors Degree in Nursing preferred
• Current RN license in state of practice
• BCLS required

Skills & Abilities

• Strong critical thinking and decisive judgment
• Ability to work with minimal supervision in a fastpaced or stressful environment
• Proficiency with multiple clinical software systems
• Effective English communication (read write speak understand)
• Ability to complete the Skills Competency Checklist within 90 days of hire

Additional Requirements

• Ability to serve diverse patient populations: Neonate through Older Adult
• Access to demographic clinical insurance financial and full medical record information
• Possible occupational exposure to bloodborne pathogens
• Physical requirements include frequent sitting standing walking keyboard use and occasional lifting up to 25 lbs
