Provide supports for children and adults in Chittenden County who live with severe and persistent mental illness. Work directly with clients in both an office environment and in field-based work in the community. The RN is a vital member of a multidisciplinary team dedicated to providing integrated community-based care. Job responsibilities include health screenings medication management and chronic disease education to individuals with mental illness substance use disorders and co-occurring health conditions. The ideal candidate must be able to work autonomously as well as in a team modality. Excellent verbal and written communication skills are essential for success.
Responsibilities:
- Complete nursing assessments contribute to interdisciplinary care planning and evaluate progress to ensure effective outcomes.
- Coordinate medication management including pick-up and delivery to individuals receiving services throughout a variety of community locations.
- Conduct health assessments and collaborate with members of treatment team to ensure access to preventative care.
- Review current medication management protocols and update as needed.
- Gather sufficient information from community partners to ensure effective holistic health care is provided.
- Coordinate health care services and provide appropriate referrals for medical and dental needs as needed.
- Maintain all necessary documentation and clinical records.
- Prepare perform and send out labs including blood draws and urine drug screens.
- Participate in peer review quality assurance and other committees as related to promoting clients health and wellbeing.
- Support and assist with coverage for nursing team members as needed.
Requirements:
- Graduation from an RN education program
- Unencumbered Vermont RN license or eligible for licensure in VT
- Valid drivers license and use of a personal vehicle
- One year of nursing experience working with individuals who have severe and persistent mental illness and/or working on an ACT team
- Capable in the use of applicable computer software including electronic medical records and basic Microsoft products.
Structure:
- Full time (37.5 hours per week)
- Exempt
- AFSCME Union (dues will apply)
- Annual salary starting at $68352.36 ($35.05/hr)
- We offer a competitive salary commensurate to experience
- This position offers the opportunity for additional compensation through On-Call coverage
