The Tennessee Department of Healths Division(TDH)of Family Health and Wellness(FHW) is hiring a contract Public Health Program Director 1(PHPD1)to lead theexpansion of the CHANT Welcome Home project and acquisition of call center and case management systems funded by theCentersfor Medicaid and MedicareServicesRuralHealth Transformation(RHT)Program. This position will provide strategic leadership and oversight forthe Welcome Home kitprojectin countiesdesignatedas maternity care deserts and act as a liaison between internal technical teams and software vendors.

This roleis responsible foroverseeing contracts guiding implementation of fundedactivities and supporting community-basedpartnersworking to improvematernal and infant outcomes. This position will report to theDeputy DirectorForChild Health.

Key Responsibilities:

• Provide leadership and oversight for theenhanced care coordination and Welcome Home Kit projectacrossdesignatedruralcounties

• Manage andmonitorgrant-funded contracts including reviewing scopes of work budgets expenditures and deliverables to ensure compliance with State and federal requirements

• Serve as theprimaryliaison to funded grantees providing technicalassistance guidance and ongoing support to ensurethesuccessful implementation of program activities

• Monitor program performance through shared measurement approaches including reviewof process and outcome data quarterly reports and evaluation activities

• Collaborate with epidemiologists and other program staff to design and implement program evaluation activities

• Work closely with Clinical Application Coordinatorsin procurement and development of new software platforms

• Support development of sustainability plans including identifying funding opportunities and strategies tomaintainprogram impact beyondthe grant period

• Conduct site visitsfacilitatemeetings and represent the program in regional and statewide stakeholder engagements

• Collaborate with internal TDH programs and external partners to align efforts and advance maternal fetal and infant health priorities

MinimumRequirements:

• Graduation from an accredited college or university with a bachelors degree in public health orarelated field

• Experience equivalent to substantial (5 years or more) of full-time increasingly responsible professional health program work

• Experiencein program or grant management including contract oversight budgeting and reporting

• Demonstrated ability to lead or support multi-partner initiatives or community collaboratives

• Strong understanding of public health data and experience applying data to program planning and evaluation

• Excellent organizational project management andproblem-solvingskills

• Strong communicationand interpersonal skills with the ability to engage diverse stakeholders including community members and individuals with livedexperience

• Experienceproviding training or technicalassistanceis preferred

• Knowledge of maternal fetal and infant healthand rural health systems is preferred

• Ability to travel within assigned regions and statewide as needed

• Ability to workremotelywith periodic in-person meetings in Davidson County andassigned rural locations

In-Office Requirements:
1. New hires will be required to come in for the first day of employment for laptop deployment and onboarding in Nashville and at least monthly or as requested by hiring manager.
2. Travel to the Nashville office will not be reimbursed.

Typical working hours: Monday-Friday 8AM to 4:30PM 37.5 hours per week
