Job Summary

Since 1957 weve assisted a wide array of populations throughout Southwestern Pennsylvania focusing on those generally underserved by traditional services including senior citizens veterans individuals with physical and/or intellectual disabilities individuals on the autism spectrum and youths who have aged out of foster care.
JOB SUMMARY
The MyPlace PTI Program offers transitional housing for young adults ages 18-23 who are currently in foster care or have aged out of foster care through CYF. The PTI Case Manager will work closely with participants to engage and offer intensive case management services. The Case Manager will focus on helping and guiding the young adults to achieve self-sufficiency and permanent housing.
ESSENTIAL FUNCTIONS
  • Developing and maintaining positive relationships with program participants
  • Teach and model independent living skills
  • Provide case management duties including school employment referrals and contacts
  • Maintaining a physically and emotionally safe environment
  • Meet with clients in their units in the office and out in the community several times a month
  • Attend and participate in monthly life skills classes with My Place staff and clients
  • Help in securing housing for program participants by visiting apartments throughout the Pittsburgh area and ensuring the cleanliness and safety of the apartment
  • Act as the Landlord by collecting rent payments from participants during the summer months and completing monthly unit inspections
  • Develop monitor and revise in conjunction with client the individuals goal plan housing plan and monthly budget
  • Assist with career development by
    • Identifying college major/minor
    • Identifying plans for use of degree
    • Providing connections to internships
    • Providing connections to professional development programs
  • Develop with client a discharge plan to include provision of two-month aftercare plan services for program graduates
  • Assist in securing permanent housing before program discharge
  • Document activities of casework and interaction with clients through written case notes
  • Agencies
    1. Establish/maintain relationships with ILP agencies in Allegheny County.
    2. Establish/maintain relationships with other service providers as needed to discharge job responsibilities.
  • Reports
    1. Prepare all reports as required by funding agencies.
  • Other responsibilities as assigned.
QUALIFICATIONS AND SKILLS
  • A degree in Social Services required.
  • Experience in youth work required.
WORK ENVIRONMENT AND PHYSICAL DEMANDS
  • Field and office
  • Involves significant local travel
  • Some evening and weekend hours
