Psychiatric Social WorkerIntake Coordinator

The Psychiatric Social Worker/Intake Coordinator will join the Ellis Outpatient Mental Health Services team. The mission of Ellis Outpatient Mental Health Services is toprovide person-centered holistic recovery-oriented care so individuals can achieve their life employee will have the opportunity to provide direct care and advocacy for a diverse and vulnerable specialize in a wide range of treatment interventions including trauma-informed care and integrated mental health and substance use treatment.

The Psychiatric Social Worker/Intake Coordinator is responsible for managing the influx of new patients across multiple referral sources along with providing direct care to patients. This position is devoted to the ongoing oversight of the intake track in the assigned outpatient program(s).The remaining time is spent as a Psychiatric Social Worker providing thorough and clinically appropriate social work interventions this includesindividual and group psychotherapypsychosocial assessments diagnosing person-centered treatment planning crisis intervention and family/collateral engagement.

EDUCATION AND EXPERIENCE REQUIREMENT(S):

• Masters Degree in Social Work LMSW licensure required LCSW preferred

PRIMARY RESPONSIBILITIES OF THE POSITION:

Intake Coordinator Responsibilities:

• Reports to Clinical Manager of Outpatient Services and the Clinical Supervisors; carries out duties as assigned
• Ensures that referrals are scheduled within established time frames triaging and prioritizing patients based on acuity/need
• Maintains accurate databases and keeps clinical notes for all referrals including utilization of Powerchart Mental Health App Soarian Financials and Referral Manager
• Ensures that proper referral information is received and screens for appropriateness of referrals
• Ensures that Intake files are converted to active medical records upon admission to clinic
• Manages the prioritization and assignment of patients to clinicians including leading a weekly meeting with the clinical supervisors
• Tracks all open intakes including managing prescriber and social worker appointments
• Provides clinical supervision in conjunction with the social work supervisors to staff clinicians on intake-related matters
• Make the final determination on which referrals are prioritized and put through for intake appointments
• Acts as liaison to other community agencies in intake-related issues including attending discharge meetings with inpatient crisis Office of Community Services and corrections departments to coordinate high need patients

Psychiatric Social Worker Responsibilities:

• Reports to Clinical Manager of Outpatient Services and the Clinical Supervisors; carries out duties as assigned
• Provides care/coordination to a diverse population of people diagnosed with Serious and Persistent Mental Illness (SPMI)
• Provides psychotherapy and psychoeducation based on the treatment need of each individual patient including providing evidence-based treatments such as CBT DBT ACT psychodynamic psychotherapy and other clinically appropriate interventions. Option to facilitate clinical groups including trauma and substance use-related groups
• Advocates for an underserved population through coordination with primary care care management and other community partnerships.
• Assess determinants of health and make referrals for appropriate services including housing transportation medical providers and care management.
• Attends individual supervision (frequency determined by supervisor) bimonthly group supervision and/or weekly team meetings with the MD/NPP staff.
• Facilitates and/or attends patient-centered meetings with community providers required for optimal patient care
• Provides intake assessments on new patients and makes formal diagnoses and recommendations that will guide further treatment at the clinic
• Utilizes the CASE approach to suicide/homicide risk to assess safety and provide accurate interventions to mitigate harm to patient and community
• Submits all documentation within 72 business hours and ensure yearly treatment plans are up to date with each billable service
• Coordinates treatment with nursing and MDs/NPPs to ensure high quality patient care as needed
• Provides clinical services and administrative support to patients who are co-enrolled in other community services and systems

Salary Range:$29.44-$44.16/hour Pay is based on experience skills and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
