Job Summary

Cordial Health is seeking a compassionate and skilled Psychiatric Mental Health Nurse Practitioner (PMHNP) or Physician Assistant with psychiatric experience to provide on-site behavioral health care throughout Queens New York.

This full-time position offers a stable weekday schedule meaningful clinical work and the opportunity to build lasting relationships with an underserved adult and geriatric population.

Responsibilities include:

  • Conduct comprehensive psychiatric evaluations and diagnostic assessments
  • Provide ongoing psychiatric consultations and follow-up care
  • Diagnose and manage psychiatric and behavioral health conditions
  • Develop individualized evidence-based treatment plans
  • Prescribe monitor and optimize psychotropic medications
  • Evaluate medication effectiveness side effects and behavioral stability
  • Collaborate with physicians nursing staff therapists social workers and families
  • Participate in interdisciplinary treatment planning
  • Educate residents caregivers and staff regarding diagnoses and interventions
  • Provide compassionate trauma-informed and resident-centered care
  • Maintain accurate timely and compliant clinical documentation


Requirements
  • Board-certified PMHNP-BC or Physician Assistant with significant psychiatric experience
  • Active and unrestricted New York professional license
  • Active DEA registration required
  • Strong knowledge of psychiatric assessment diagnosis psychopharmacology and treatment planning
  • Ability to manage a psychiatric caseload independently
  • Excellent communication organization and relationship-building skills
  • Geriatric skilled nursing rehabilitation long-term care or post-acute experience preferred
  • Familiarity with electronic medical record systems preferred

  • Required Skills:

    Valid Nurse Practitioner license in the state of New York with specialization in Psychiatry (PMHNP). Ability to work independently and effectively manage a caseload of patients. Strong diagnostic skills and knowledge of psychiatric medications and treatment modalities. Excellent interpersonal and communication skills to establish rapport with patients families and interdisciplinary team members. Proficient in electronic health record (EHR) systems and basic computer skills. Compassionate patient-centered approach to care.
