As part of a multidisciplinary team this role delivers integrated recovery-oriented care and collaborates with providers families schools and community partners to support individuals with complex behavioral health needs across clinic and community settings. Youth Act Psychiatric Nurse Practitioner Job Responsibilities:Conduct psychiatric evaluations diagnostic assessments medication management and ongoing treatment for children adolescents and adults including prescribing psychotropic medications and Medication-Assisted Treatment (MAT) when and monitor individualized treatment plans review diagnostic findings provide client and family education and coordinate care to support positive clinical and manage psychiatric crises including suicide risk assessments safety planning crisis stabilization and rapid follow-up in accordance with Zero Suicide with multidisciplinary teams Youth ACT staff primary care providers families schools and community partners to coordinate integrated recovery-oriented care across clinic and community timely accurate clinical documentation and ensure compliance with OMH OASAS Medicaid regulatory payer and organizational standards while participating in quality improvement and utilization management productivity licensure certification and professional development requirements; participate in staff meetings respond to emergencies report untoward incidents and perform other duties as assigned. Youth Act Psychiatric Nurse Practitioner Qualifications:Masters or Doctoral degree in Nursing from an accredited program with current New York State Nurse Practitioner Psychiatric Mental Health Nurse Practitioner (PMHNP) certification and DEA registration providing psychiatric services in outpatient and/or community-based behavioral health settings with children adolescents adults and families with complex mental health and substance use disorders clinical assessment diagnostic medication management and treatment planning skills with knowledge of recovery-oriented and trauma-informed care communication interpersonal and collaboration skills with the ability to work effectively within a multidisciplinary team and manage competing with electronic health records and clinical documentation with a commitment to culturally responsive person-centered :$150000 Annually#INDIHC4Liberty Resources is committed to creating a diverse inclusive environment and is proud to be an equal opportunity employer.
