Title:Program Manager - Targeted Case Management (TCM)
FLSA Status:Exempt
Purpose:
The Program Manager oversees Guilds Targeted Case Management (TCM) program. Our TCM Case Managers provide person-centered care to clients in the community by coordinating and facilitating services that provide individual support and resources to maintain and enhance mental health and independence.
This position provides operational leadership and clinical supervision to Program Supervisors and Case Managers while also managing overall team service delivery financial structures integration and expansion opportunities.
Key Responsibilities:
Duties and Responsibilities:
- Provide leadership to Targeted Case Management team including clinical and operational oversight.
- Manage multiple program contracts ensure compliance with reporting requirements and stay informed on laws statutes and policies impacting participant services.
- Oversee program budgeting ensuring accountability and good financial health for the TCM program.
- Supervise hire train and manage staff performance to align with agency values mission and culture.
- Lead initiatives to promote diversity respect and cultural competence in the workplace.
- Collaborate with community partners government agencies and other stakeholders.
- Perform other duties as needed to support client care and department operations.
Required Skills/Abilities:
- Knowledge of Severe and Persistent Mental Illness (SPMI) and the needs of individuals with mental illness.
- Ability to develop and maintain receptive open and approachable lines of communication with clients and staff and provide a friendly environment for visitors.
- Ability to interpret and evaluate data/reports and respond appropriately making any needed program adjustments.
- Demonstrated skill in management and supervision of staff.
- Strong written and verbal communication skills including composing proofreading and editing.
- Presentation and meeting facilitation skills.
- Flexibility to adapt to changing conditions and openness to learning new processes.
- Ability to work as a proactive and positive member of a team to deliver quality services.
- Ability to maintain confidentiality of organization personnel information and client data.
- Ability to use sound judgment and maintain healthy professional boundaries.
- Analytical and problem-solving skills.
- Ability to meet deadlines and set priorities.
- Proficiency with a variety of web-based software and databases including Microsoft Office Suite and electronic health record systems.
Working Conditions:
- This position is based at Guilds St. Paul office and requires some client visits in the community.
- This position requires occasional transportation of clients in personal vehicle.
- This position has the potential to encounter bodily fluids communicable diseases tobacco smoke and/or household pets.
- Some activities may be required in the community.
- Travel may be required to other program sites.
- Prolonged periods at a desk working on a computer.
- Manual dexterity for tasks such as taking notes using technological devices for documentation or assisting clients with certain activities if needed.
- Must be ambulatory and able to negotiate stairways.
- The ability to lift and carry heavy weights that could be up to 25 pounds.
Education and Experience:
Required:
- Masters degree in the behavioral sciences.
- Licensed as an independent mental health professional (LICSW LMFT LPCC)
- Board approved supervisor with licensing board.
- 5 years of experience overseeing and providing services to people with Severe and Persistent Mental Illness.
Preferred:
- Experience supervising a Targeted Case Management team.
Additional Requirements:
- Must not be actively receiving services from Guild and must not have received services within the last two years.
- A valid Class D Drivers License reliable vehicle satisfactory driving record and necessary insurance.

The salary range for this position is between $80000 to $100000 annually. The range reflects the minimum and maximum salary for the position. Compensation will depend on the candidates professional experience skills and relevant education training and certifications.
