Position Summary
Coordinate program operation and administrative tasks as needed. Provide clerical administrative member and team support for the Diabetes Prevention Program. The goal of the program is to prevent or delay type 2 diabetes by supporting members. Interacts with internal departments to accomplish program goals and track member progress. Helps coordinate member enrollment quarterly check ins and other data documentation as required by the CDC. This is a FT WFH role.
Pay Range
$23.34- $26.26 hourly (depending on experience)
Actual pay is based on qualifications. Applicants who do not exceed the minimum qualifications will only be eligible for the low end of the pay range.

Required Skills Experience & Education:
- Bachelors degree in health promotion preferred or equivalent work experience in the health care industry.
- Experience in program support ability to track & coordinate multiple projects with extended timelines and healthcare operations preferred.
- Ability to work collaboratively and independently.
- Excellent communication interpersonal and presentation skills.
- Strong cultural competence and understanding of the importance of using Diversity Equity and Inclusion (DEI) concepts to guide and inform work.
- Thorough knowledge and understanding of health behavior change theory.
- Experience with Microsoft Office products.
- Ability to learn proprietary databases and maintain confidentiality.
- Experience in health coaching or field related to health coaching.
- Experience in health insurance.
- Speaker of both English and Spanish.
- Maintains a professional appearance and demeanor in all internal or external interactions.
Primary Functions:
- Supports the coordination and implementation of Diabetes Prevention Program.
- Conducts outreach and new member enrollment with Pre-D program eligible members via phone email mail and virtually to support member recruitment enrollment retention and completion of the Pre-D program.
- Assists care team with general clerical support and program operations.
- Routinely documents member progress records and verifies the required member data to submit externally (CDC employer groups) and report internally to various committees (Medical Quality Improvement Committee Quality Steering Council etc.) Supervisor and other leaders as needed.
- In collaboration with the Supervisor and data team ensures all data is submitted accurately and timely.
- Develops and monitors various key performance indicators including outreach enrollment attendance retention completion disenrollment and health outcomes.
- Manages program calendar emails and telephone lines.
- Complies with all applicable laws and regulations including those governing participant privacy and data security.
- Ability to work a flexible schedule including early mornings over the lunch hour and early evenings to meet the needs of program participants.
- Performs other job-related duties as assigned.
Working Conditions &Contact with Others
- Office environment with extensive close PC and keyboard work constant sitting and phone work. Must be able to navigate multiple screens. Work in excess of standard work week in order to meet business needs.
- Internally with all departments and subsidiary companies. Externally with various groups and banks.
