Make a meaningful difference while enjoying the flexibility of a PRN schedule. As a Home Health Speech-Language Pathologist youll provide personalized one-on-one care to patients in the setting where they feel most comfortabletheir own homes. Youll assess and treat communication cognitive and swallowing disorders while educating patients and caregivers on techniques that support safety independence and quality of life.
- Evaluate patients speech language cognitive communication and swallowing needs.
- Develop individualized treatment plans based on physician orders and assessment findings.
- Provide skilled therapy services in patients homes.
- Teach patients and caregivers exercises techniques and strategies to support continued progress.
- Recommend and provide instruction on assistive devices when appropriate.
- Monitor patient progress and update treatment plans as needs change.
- Communicate changes in condition progress and recommendations to physicians and the interdisciplinary care team.
- Coordinate services with nurses therapists caregivers and other healthcare professionals.
- Complete accurate and timely clinical documentation.
- Maintain compliance with professional standards regulatory requirements and company policies.
- Masters degree in Speech-Language Pathology from an accredited program.
- Current unrestricted Speech-Language Pathologist license in the state of practice.
- At least one year of SLP experience in a clinical care setting.
- Valid drivers license and an acceptable driving record or reliable transportation to travel between patient homes and assigned locations.
- Sit stand bend lift and move intermittently and be able to lift 50100 lbs.
- Flexible PRN scheduling.
