We are seeking a dedicated and compassionate Primary Care Provider (MD DO NP or PA) to join our dynamic team. The ideal candidate will provide comprehensive outpatient care build long-term relationships with patients and contribute to our mission of delivering high-quality healthcare in a personalized setting. This can be either a full-time or part-time position.
Responsibilities include:

- Providing preventive acute and chronic care management for patients of all ages.
- Collaborate with a multidisciplinary team to ensure continuity of care.
- Maintain accurate and timely documentation in our EMR system.
- Participate in quality improvement initiatives and practice development; and foster a welcoming and respectful environment for patients and staff.
- Must be licensed (or eligible for licensure) in the state of Vermont.
- Have strong communication and interpersonal skills.
- Have a commitment to patient-centered care and community health.
- Experience in outpatient primary care preferred.
