Were committed to building a diverse workforce that values contributions of all team members and work to recognize cultural beliefs values traditions language preferences and health practices of the communities that we serve and how they apply to provision of positive health the direct supervision of the Prevention Supervisor the Caseworker is primarily responsible for providing case management and counseling services toidentifiedyouth and include advocacy referral and linkage group individual and familycounselingand crisis Job FunctionsUtilizingthe task centered case management model implements and coordinates services to children and theirfamilyshaving troublein the home and/or in school aimed at preventing placement of children outside the home and/or at improving academic intervention and support to families in crisis intended to keep the family unit all required documentation in timelinesin accordance withprogram and ResponsibilitiesAssesses children and families for risk develops and implements service plans to address risk factors drafts petitions and testifies in FamilyCourtas Foster Care Services andlocatesplacements when on behalf of children and their families to ensure adequacy of services (i.e. educational legal and community) by providing linkage and coordination with thecountyproviders and and reviews intake of all referrals to services in a manner which is culturally competent. Empowers clients by using clientscultural strengths and informal support networks in service effective professional relationships with other service providers and community organizations in the interest ofclientsand program excellence and responsibility seeing opportunity where others see barriers to success. We strive to create and sustain an inclusive environment where everyone feelsa strong senseof Untoward Incidents out Agency policies and all necessary written reports statistics casefilesand summariesin a timely mannerandin accordance withprogram and agency andparticipatesin individual supervisory conferences staff meetings trainingsessionsand professional development opportunities as required program door-to-door transportation as welfare of passengers during loadingunloadingand use of personalvehiclefor program transportation as emergency situations which may require flexibility of time and area of other dutiesresponsibilitiesand related tasks as two (2)years experienceworking in a school- and/or home-based setting providing services to individuals and/or working with families and their have valid New York State drivers license and access to reliable Skills and AbilitiesStrong oralwrittenand public speaking communication alsopossessstrong problem solving decision making and conflict resolution skills andexhibita high levelofself-confidenceand be able to work with complex systems (ex. Probation Family Court Foster Care) and have knowledge and/or experience with both the child welfare and juvenile justice to work independentlyand with flexibility indays and hours of doing home-based work in a rural setting have access to reliable transportationand havea valid drivers : $23.08 Per Hour
