Practice Transformation Specialists serve as Aledades frontline champions building meaningful relationships with healthcare providers while gaining invaluable insights into our partner practices evolving needs and success stories. As part of our innovative Flex Field Practice Transformation team youll deliver scalable expert-level support that ensures every practice receives the consistent high-quality guidance they need to thrive in value-based care. This dynamic role puts you at the heart of healthcare transformation where your direct impact helps practices achieve better patient outcomes while driving meaningful change across the healthcare landscape.

$75000 - $90000 a year

Aledade PBC a public benefit corporation exists to empower the most transformational part of our health care landscape - independent primary care.
