Designated as the Social Care Network (SCN) Lead Entity for the Capital Region Central NY and North Country under New Yorks 1115 Waiver Amendments SCN & Health-Related Social Needs (HRSN) Program we are responsible for ensuring that there is a seamless consistent end-to-end process for screening navigation and the delivery of services including food housing and transportation to thousands of Medicaid Members. Who You Are
ThePopulation Health and Informatics Analyst establishes and maintains analytic processes that support Healthy Alliances mission population health strategy community health work programs and health-related social need (HRSN) services. This role turns clinical social operational and community-based data into clear insights that help leaders and teams identify needs monitor performance and improve outcomes. Success in this position requires strong data integrity practical analysis and the ability to translate information into action for population health operations community health workers and regional teams. The role helps Healthy Alliance direct resources where they can create the greatest impact for members and communities.
What Youll Do
- Analyze clinical social operational and community-based data to identify opportunities for intervention improved engagement and better outcomes.
- Develop dashboards recurring reports scorecards and ad hoc analyses that support population health priorities HRSN performance regional and network operations and leadership decision making.
- Monitor contractual/value-based contract performance related to HRSN services engagement referral activity service completion utilization outcomes and community health worker program performance.
- Support community health work programs by reporting outreach engagement referrals follow-up activity care coordination and community-based outcomes.
- Assist population health leaders in evaluatingcommunity health worker deployment models across regions populations partners and intervention types.
- Translate business and program questions into reporting requirements analytic specifications and user-friendly outputs.
- Validate reconcile and interpret data from multiple internal and external systems with a strong focus on data quality and consistency.
- Partner with technology operations and program teams to improve data capture documentation consistency and reporting workflows.
- Produce clear summaries visuals and recommendations that help operational leaders understand trends barriers and improvement opportunities.
- Support grant reporting contractual reporting payer-facing reporting value-based care readiness efforts and external stakeholder updates as needed.
- Contribute to organizational learning by identifying what is working where outcomes are improving and where additional support or redesign may be needed.
- Perform other responsibilities and duties as assigned.

What Youll Need
Education
- Bachelors degree in public health health informatics health administration data analytics statistics information systems or a related field required. Equivalent work experience in a related field may be considered in lieu of degree requirements.
Professional Work Experience
- Three to five years of experience in health care public health social care population health informatics reporting or data analytics required.
- Experience working with health care community-based operational or population-level datasets required.
- Experience with Medicaid managed care health-related social needs Social Care Networks community health worker programs care coordination or care management preferred.
Knowledge Skills and Abilities
- Strong proficiency with Microsoft Excel and experience using business intelligence or reporting tools.
- Experience with Power BI SQL Tableau or similar analytic tools preferred.
- Ability to communicate findings clearly to both technical and non-technical stakeholders.
- Strong analytical thinking problem-solving skills attention to detail and commitment to data integrity.
- Understanding of health equity social drivers of health and community-based interventions preferred.
- Ability to work collaboratively across Population Health Operations Technology Finance and community-facing teams.
