Job Description Summary

How will you make an impact & Requirements

• Perform comprehensive patient evaluations including history physical examination and diagnostic interpretation.

• Diagnose and manage acute chronic and complex medical conditions.

• Develop individualized treatment plans and monitor patient progress.

• Order interpret and follow up on diagnostic tests (labs imaging PFTs cognitive screenings cardiac assessments etc.).

• Provide preventive care including immunizations screenings and lifestyle counseling.

• Manage medication therapy including prescribing monitoring and reconciliation.

• Participate in care gap closure chronic disease management and preventive health initiatives.

• Utilize evidencebased guidelines to improve patient outcomes and reduce avoidable hospitalizations.

• Document care accurately to support quality reporting risk adjustment and compliance.

• Engage in population health strategies including outreach followup and care coordination.
