Job Description:

We are seeking a compassionate and experienced Family Medicine Physician Assistant to join our dynamic team at Foundation Medical Partners / Southern NH this role you will play a vital role in providing comprehensive care to a diverse patient population in Milford NH.

• Collegial practice with solid APP support
• W2 employed position with Foundation Medical Partners
• Not-for-Profit 501(c) Organization
• 40 Hours 50% direct patient care work and 50% coordinating and communicating patient care
• Clinical Affiliate of Massachusetts General Hospital
• Hybrid Work Schedule Potential remote options once fully trained

Qualifications:

• Certified Physician Assistant
• 2 years of Family Medicine experience
• Strong clinical skillsincluding assessmentdiagnosisand treatment planning.
• Excellent communicationcollaborationand interpersonal skills.
• Proficiency with EPIC EMR preferred
• Ability to work independently and as part of a team.
