We pride ourselves on high quality integrated community-based services that meet the evolving needs of our Assistant (PA) Position Summary:Liberty Resources is seeking applicants for a Physician Assistantfor our Integrated Health Center in Syracuse collaboration with the agency Medical Director the Physician Assistant will function as a member of theIntegrated Health CareTeam in assisting with a full range of nursing/medical a team-based approach patient centered care is provided to patients of all Assistant (PA) Job Responsibilities:Performs interviews health histories physical examinations mental status assessments and diagnostic tests to determine diagnosis and current health status of medical crisis intervention/counseling as required by direct visitation or telephone evaluations medication reviews and care plans reviews and progress in the management of clients medication regimens; prescribes drugs and medication treatments as indicated by diagnosis lab results and consistent with community standards of all appropriate primary care care coordination and medication education to clients and to the treatment planning process and crisis intervention services when health education to clients and families to support collaboratively with mental health staff to provide integrated healthcare for all clientsLiberty Resources Clinics are approved sites for the National Health Service Corps Loan Repayment Program (Tier 3).Physician Assistant (PA) Qualifications:Certificate and current registration to practice as a Physician Assistant in New York least one year experience providing direct services in a family medicine with individuals diagnosed with mental illness or comparable experience is have eligibility for full and unconditional participation in the Medicaid and Medicare use an EMR and complete all required documentation in a timely professional be flexible and oriented to family to cultural differences and able to incorporate that understanding into service provision.
