Physical Therapy Case Manager Home Health

• Hours: 40 hours per week

This is a Home Health Physical Therapist Case Manager position supporting patients in the Central Metro A territory. The PT will manage their own caseload provide skilled home-based therapy and coordinate care to help patients safely recover and maintain independence at home.

This role offers a steady weekday schedule with no on-call weekends or holidaysideal for therapists who value consistency and autonomy.

• Perform in-home physical therapy evaluations and treatments

• Develop and implement individualized plans of care

• Act as the case manager for assigned patients coordinating services as needed

• Educate patients and caregivers on mobility safety and home exercise programs

• Complete timely and accurate documentation per home health standards

• Communicate effectively with physicians nurses and interdisciplinary team members

• Ensure compliance with regulatory and agency requirements

• Adult and geriatric home health patients

• Typical diagnoses include post-acute recovery mobility impairments and chronic conditions requiring skilled PT

• Active Physical Therapist license in Illinois

• Home Health PT experience strongly preferred

• Strong clinical judgment and time-management skills

• Comfortable working independently in a home-based setting

• No floating required

• Coverage limited to the assigned Central Metro A home care territory
