We are seeking a compassionate and motivated Physical Therapist Assistant (PTA) to join our rehabilitation team on a PRN basis. Under the direction of a licensed Physical Therapist the PTA will provide skilled therapy services designed to help residents improve mobility strength function and independence while enhancing their quality of life.

• Provide physical therapy treatments as directed by the supervising Physical Therapist
• Follow physician orders and individualized treatment plans
• Document daily treatments weekly progress notes and resident outcomes accurately and timely
• Participate in patient care conferences and rehabilitation meetings as needed
• Monitor resident progress and communicate updates to the Physical Therapist
• Assist with cleaning and maintenance of therapy equipment and treatment areas
• Train and assist Restorative Aides in carrying out therapy-related programs
• Communicate effectively with residents families nursing staff and interdisciplinary team members
• Participate in staff education and in-service training programs
• Educate residents families and caregivers on maintenance programs and discharge recommendations
• Report equipment concerns and assist in maintaining a safe therapy environment
• Ensure compliance with all facility policies state regulations and payer requirements

• Graduate of an accredited Physical Therapist Assistant program
• Experience in skilled nursing rehabilitation or long-term care preferred
• Experience with PointClickCare (PCC) and Casamba preferred
• Strong communication documentation and organizational skills

• Current South Carolina Physical Therapist Assistant (PTA) license in good standing
• Must maintain all required continuing education requirements
• Must remain in good standing with the applicable State Board
