Remedy Home Health Care is actively seeking a licensed Physical Therapist (PT) to join our growing Home Health team in Ventura County. This Per Diem Physical Therapist role offers flexibility autonomy and the opportunity to provide high-quality rehab and community-based care directly to patients in their homes.

As a Home Health Physical Therapist you will work with senior care populations including geriatric patients recovering from illness injury or surgery. Youll deliver skilled therapy services in home-based assisted living and community care settings helping patients regain independence and improve their quality of life.

As a Physical Therapist in Home Health you will provide skilled rehab services tailored to each patients needs. Your focus will include fall prevention strength building mobility training and functional independence for senior and geriatric patients.

• Conduct in-home physical therapy evaluations and reassessments
• Develop and implement personalized rehabilitation care plans
• Improve strength balance mobility and functional outcomes
• Provide therapy in home health assisted living and community care settings
• Educate patients and families on fall prevention and safe mobility techniques
• Document visits using WellSky (Kinnser) EMR
• Collaborate with interdisciplinary home health teams

• Licensed Physical Therapist (PT) California (Required)
• CPR Certification (AHA approved)
• Minimum 1 year of clinical experience (Home Health preferred)
• Experience in senior care geriatrics rehab or community-based care preferred
• Strong clinical communication and patient education skills
• Valid drivers license reliable transportation and auto insurance
