San Bruno Skilled Nursing is seeking a compassionate and motivated Physical Therapist (PT) to join our full-time in-house rehabilitation team.

We are committed to providing high-quality resident-centered therapy that helps individuals improve mobility regain independence and achieve meaningful functional goals. Whether youre an experienced clinician or a recent graduate youll have the opportunity to work in a collaborative environment with support mentorship and a strong focus on resident outcomes.

Position Responsibilities

  • Evaluate residents and develop individualized physical therapy treatment plans.

  • Provide skilled physical therapy services to improve mobility strength balance coordination and functional independence.

  • Establish measurable treatment goals based on each residents needs and abilities.

  • Monitor resident progress and adjust treatment plans as appropriate.

  • Document evaluations treatment sessions progress and discharge recommendations accurately and timely.

  • Collaborate with nursing occupational therapy speech therapy physicians and other members of the interdisciplinary team.

  • Educate residents families and caregivers regarding therapy programs safety mobility and home exercise programs.

  • Supervise and provide clinical guidance to Physical Therapist Assistants (PTAs) as applicable.

  • Promote resident safety dignity independence and quality of life.

Qualifications

  • Current and active California Physical Therapist (PT) license required.

  • Strong clinical assessment treatment communication and documentation skills.

  • Ability to work effectively as part of an interdisciplinary team.

  • Passion for geriatric rehabilitation and improving functional outcomes.
