San Bruno Skilled Nursing is seeking a compassionate and motivated Physical Therapist (PT) to join our full-time in-house rehabilitation team.
We are committed to providing high-quality resident-centered therapy that helps individuals improve mobility regain independence and achieve meaningful functional goals. Whether youre an experienced clinician or a recent graduate youll have the opportunity to work in a collaborative environment with support mentorship and a strong focus on resident outcomes.
Position Responsibilities
Evaluate residents and develop individualized physical therapy treatment plans.
Provide skilled physical therapy services to improve mobility strength balance coordination and functional independence.
Establish measurable treatment goals based on each residents needs and abilities.
Monitor resident progress and adjust treatment plans as appropriate.
Document evaluations treatment sessions progress and discharge recommendations accurately and timely.
Collaborate with nursing occupational therapy speech therapy physicians and other members of the interdisciplinary team.
Educate residents families and caregivers regarding therapy programs safety mobility and home exercise programs.
Supervise and provide clinical guidance to Physical Therapist Assistants (PTAs) as applicable.
Promote resident safety dignity independence and quality of life.

Qualifications
Current and active California Physical Therapist (PT) license required.
Strong clinical assessment treatment communication and documentation skills.
Ability to work effectively as part of an interdisciplinary team.
