Concept Rehab is seeking a Physical Therapist (PT) to join our skilled nursing rehabilitation team in Johnston Iowa.

Key Responsibilities

• Evaluate patients following physician referral and establish individualized plans of care
• Develop functional treatment goals that promote mobility and independence
• Deliver skilled physical therapy interventions in accordance with evidence-based practice
• Assess gait balance strength endurance transfers and functional mobility
• Supervise and collaborate with Physical Therapist Assistants (PTAs)
• Communicate patient progress with physicians nursing staff families and interdisciplinary team members
• Participate in care conferences and discharge planning meetings
• Educate patients and caregivers regarding safety mobility and home exercise programs
• Maintain accurate and timely clinical documentation
• Support quality outcomes and patient satisfaction initiatives

Qualifications

• Graduate of an accredited Physical Therapy program
• Current Iowa Physical Therapist license or eligibility for licensure
• Strong communication and interpersonal skills
• Ability to work collaboratively within an interdisciplinary rehabilitation team
• Passion for serving older adults in a Skilled Nursing Facility and post-acute rehabilitation setting
