Job Responsibilities:
Evaluate diagnose and treat patients with physical impairments in a post-acute setting
Develop and implement individualized treatment plans based on patient needs and clinical findings
Collaborate with interdisciplinary teams to ensure coordinated patient care and recovery planning
Supervise and work closely with Physical Therapist Assistants as needed
Monitor patient progress and modify treatment plans based on clinical outcomes
Educate patients and caregivers on mobility safety and home exercise programs
Maintain accurate and timely documentation in compliance with facility and regulatory standards
Requirements
Active state license as a Physical Therapist in Tennessee
Current BLS/CPR certification
Minimum of 1 year of recent PT experience preferred ideally in post-acute SNF or rehabilitation settings
Strong clinical reasoning and patient care skills
Ability to work collaboratively within a multidisciplinary healthcare team
