Choose the areas that work for you.

You do not have to accept patients throughout the entire territory.

• Complete physical therapy evaluations in patients homes

• Develop individualized plans of care

• Help patients improve strength balance endurance and mobility

• Provide therapeutic exercise and functional training

• Provide gait transfer and stair training

• Assess fall risks and home safety

• Help patients regain mobility following illness surgery injury or hospitalization

• Educate patients families and caregivers

• Monitor patient progress and modify treatment plans

• Complete reassessments and discharge planning

• Collaborate with the interdisciplinary care team

• Complete timely clinical documentation using WellSky

• Active Maryland Physical Therapist License Required

• Graduate of an accredited Physical Therapy program

• Reliable transportation

• Strong clinical and communication skills

• Ability to work independently in patients homes

• Home Health Experience Preferred but Not Required

• WellSky Experience is a Plus
